Who will be collecting this information C-Health Insurance LLC will collect information provided by customer to set up an account and apply for Health or Dental coverage on Virginia Insurance Marketplace website 

Personally Identifiable Information (PII) refers to any data that can be used to identify a specific individual. This can include both direct identifiers, which uniquely identify a person, and indirect identifiers, which can be combined with other information to identify someone.  

What PII is collected Full name, Social Security number (SSN), driver’s license number, passport information, and email address, date of birth, gender, Zip code and race etc. 

Examples of PII Name Address and Telephone# etc. 

Who that information will be used, for setting an account, summiting application on the on the Virginia Insurance Marketplace website/C-Health Insurance website, for Health and Dental Insurance, Qualified Heal Plan or Qualified dental Plan which may include Other Government agencies HHS, CMS, SSI and link to other Health Insurance Carriers. 

How that information will be kept secured, All Personal Identifiable Information PII will be encrypted on a secure Virtual Private Network VPN. That information will be kept securely for 10 years under federal law according to Health and Human Services, Privacy Act of 1974 (5 U.S.C. § 552a(e)(4)). 

Collection of Personally Identifiable Information PII is voluntary but necessary in order to apply for health insurance through Virginia Insurance Marketplace website/C-Health Insurance LLC website. If, however, PII is not provided customer can apply by going to www.marketplace.virginia.gov 

 If you feel that your PII has been handled improperly, you may submit a complaint by email to the C-Health Insurance LLC pgsamworks@c-healthinsurance.com or Call 443-272-5759.     also, to the Virginia Marketplace Privacy Officer at privacy@marketplace.virginia.gov. You may also call the Marketplace customer assistance center at 1-888-687-1501 (TTY 711 for those who are deaf, hard of hearing or speech disabled.
